Left click that person with alt pressed down. Then, press 1:1 battle in menu that pops up. The duel will start when the other user approved the duel.

You can also duel through typing /fight [username], or typing /confront [username]. You can duel characters level 10 and above. There is no experience or SPI loss due to battle, only reputation loss and gain.
